Regularly reassess treatment effectiveness, adjust goals as necessary, and document progress.
Communicate patient needs and progress with the care team, physicians, patients, and family members as appropriate.
Required Qualifications
Licensed Physical Therapist in the State of Maryland
Home Health Care experience with OASIS proficiency
