Responsibilities
Conduct comprehensive assessments, plan and implement individualized care, and evaluate patient outcomes in a home health environment.
Provide clinical oversight and support for well, chronic, or acutely ill patients, ensuring appropriate interventions are in place.
Coordinate communication among providers, caregivers, and interdisciplinary team members to optimize patient health and safety.
Oversee the management of medications, including ordering, reviewing, and discontinuation, and ensure compliance with policies for administration, storage, and disposal.
Report significant changes in patient condition to physicians and document all updates accurately in the health record.
Required Qualifications
Must have home health experience.
HCHB experience strongly preferred.
Wound care/wound vac and IV therapy experience preferred.
Active RN license required; compact licensure accepted.
